Skating Permission and Safety Agreement

Introduction

My participation in skating at the Ice Peaks rink in Peres Park, Holon, operated by Psagot HaKerach Ltd., is conditional upon my adherence to the skating rules and my consent to these rules (as well as consent for my minor children or children for whom I am the legal guardian), which I provide by signing this agreement. By providing my personal identification details (and those of my minor children, if applicable), I confirm that I have read and fully agree to all terms and conditions. I understand that I am under no legal obligation to provide these details, but without them, I cannot participate in skating activities at the Ice Peaks arena.

Use of the Skating Rink

I acknowledge that for safety and security reasons, access to and use of the ice rink are subject to the following conditions:

• Watching and listening to the safety instructions displayed on screens and broadcast over the public address system at the rink, and complying with them.
• Accepting the terms of the rules posted on the website and the conditions outlined in this agreement.
• Adhering to the published rules of conduct at the rink and following the safety guidelines developed by the Psagot team ("Psagot Rules").

I confirm that I have read the rules of conduct and safety instructions before signing this agreement. I commit to re-reading these rules and instructions each time I visit the rink. I also commit to following all Psagot Rules.

I understand that Psagot, at its sole discretion, may deny me entry to the rink or remove me from the premises if I fail to comply, in whole or in part, with the Psagot Rules. I also understand that access may be denied for technical reasons, by order of a competent authority, or if the rink capacity is exceeded. If I am denied entry but have a fully paid ticket, I will be given the opportunity to visit the rink at another time.

Health and Safety

• I declare that I am in good health and do not suffer from any medical condition that could be aggravated by ice skating.
• I am not pregnant.
• I am aware that entering the rink under the influence of drugs and/or alcohol is strictly prohibited.
• I acknowledge that for safety reasons, food, drinks, and animals are not permitted on the ice rink, and I agree to abide by this rule.

Responsibility and Risks

I understand and agree that using the ice rink is at my own exclusive risk and is a type of sport/leisure activity with inherent risks to the participant, including the risk of serious bodily injury, disability, and even death. I understand that while on the ice, I may fall and be injured, and that I may also be harmed by another person on the rink.

It has been explained to me that possible risks include, but are not limited to: cuts, bruises, fractures, injuries to the wrists, arms, legs, ankles, back, head, and neck, as well as spinal injuries. I voluntarily assume all risks associated with visiting the rink.

In accordance with the law, I hereby release Psagot, its executives, shareholders, managers, employees, and representatives from any and all liability for any injury to my health, losses, and/or damage to my property incurred while at the ice rink for any reason.

Photography and Video

I am aware and agree that Psagot uses cameras at the rink and that participants are filmed. I give my consent for Psagot to use, in accordance with the law and at its sole discretion, videos and photographs in which I and/or my minor children appear, and I confirm that I will have no claims, including for monetary compensation, in this regard.

General Rules

• This permission form must be signed by adult visitors (over 18 years of age).
• Minors must have a permission form signed by a parent or legal guardian.
• An electronically signed permission form is valid indefinitely, but the company may, at its discretion, request a renewal. A manually signed form is valid for a single visit only.
• I consent to the processing of my data for business, legal, and marketing purposes (in accordance with the Privacy Policy).

"Skating Aid" Accessory

Before using the "Skating Aid" accessory, I declare that I agree to the following instructions:

• It is forbidden to sit on the aid or to transport people on it.
• The aid may only be used by children up to 10 years of age and under the close supervision of an adult.
• Use of the aid is subject to the rink management's decision based on rink capacity and safety considerations.
• The rink management reserves the right to discontinue the use of the aid at any stage.
• The user is solely responsible for any harm or damage caused by improper use of the aid.
• In case of high demand, the use of the aid may be limited to 15 minutes.
